Registration for Mathcamp 2010

Your child was accepted: congratulations! The next step is to register for this summer's program. The registration deadline for most students is May 30. (There may be some exceptions: for instance, students admitted in late May off of the waitlist will have registration deadlines in mid June.)

One component of registration is the online form. Accepted students receive instructions about online registration in their admissions email.

The other component is to mail paper materials. Please submit the following:

• Mathcamp Indemnity Form and Mathcamp Medical Form (see Permission Forms)
• Tuition Deposit (25% of {the camp fee, less any scholarships}), in a check or money order made out to Mathematics Foundation of America
• If receiving a scholarship, the front page of your 2009 Form 1040 or equivalent. It suffices to send only the front page including the adjusted gross income. These forms will be stored in a cabinet accessible only to Mathcamp staff, but you may prefer to black out social security numbers for extra security.

The mailed package must be received (not postmarked!) by the registration deadline. Our offices are not always staffed during business hours, so we are unable to accept packages that require a signature. Please do not send certified mail. Thanks!
