Permission Forms for Mathcamp 2012

Students will need several permission forms to participate in Mathcamp; some are required, others are optional. We must receive the mandatory forms by the student's registration deadline, along with the tuition deposit. The optional forms may be submitted at any time up to and including the first day of camp.

Required for registration: (All campers' required forms must be received by the registration deadline.)

• Mathcamp indemnity form [PDF]
• Mathcamp medical form [PDF]
• Model Release Form [PDF], indicating whether or not you give permission for the use of photos

Optional (as applicable):

• Permission to leave campus with relatives [PDF]
