When You Arrive at Mathcamp 2010
Day 0 at Mathcamp is very exciting! When you get to campus, you will check in at the registration desk in the dorm, and we will show you to your dorm room. Throughout the day, there will be (optional and informal) events scheduled like games and campus tours. You will also meet your Residential Advisor (to whom you will already have been introduced via email). With students arriving throughout the day, camp feels a little hectic, but your goal for Day 0 is just to get settled and comfortable.
Upon arrival at Mathcamp, all students should be prepared to submit the following items at the camp registration desk:
- The balance of the camp fee, payable in US$. Students from Canada and the US may pay by personal check, bank draft, or travelers cheques. International students are requested to pay by bank draft or travelers cheques only. Please do not pay in cash, since transporting cash always involves a risk (for you and for us). Checks, bank drafts and travelers cheques should be made payable to "Mathematics Foundation of America" (not Mathcamp).
- Proof of medical insurance. US students must bring a photocopy of their insurance card. The card should show, or be accompanied by, a complete description of the insurance policy, including name of insurance company, name of primary policy holder, policy number, and type of coverage. The accompanying description may be an official information sheet from the insurer or a description signed by a parent/guardian, and must indicate what is covered and how much is covered in dollar terms, including upper limits and deductibles. Non-US students and other students who purchased travel insurance should bring all available documentation pertaining to their policy.
- (Only if you didn't already send it in) Immunization Records, including up-to-date mumps or MMR vaccines. Alternatively, you may submit a physician's statement explaining that the student has not been vaccinated for health reasons, or a signed statement from a parent/guardian indicating that the family has religious objections to vaccines.
- (Only if you didn't already send it in) A recent photograph, preferably small, with your name written on the back. This photograph will go on the camp face-board, so it should be a picture that is easily recognizable and reasonably flattering.
- (Optional, but recommended) Signed permission forms for field trips. See permission forms.
- (Optional, but recommended) Signed model release form to give permission for the use of photographs. See permission forms.
- (Optional) Permission to take student off-campus. If there are friends or relatives who are authorized to take the student off-campus (either for a day or for an overnight visit), please have a parent or guardian complete and sign the Off-Campus Permission Form. See permission forms.
